How do I fill out the Xendit onboarding form?

Xendit is required by financial regulators to collect certain information from businesses before activating disbursement capabilities. This process helps protect both your business and your customers by preventing fraud and other financial risks.

Here's a step-by-step guide to help you fill out the form correctly and avoid delays in your verification process.

Step 1: Choose the Correct Business Type

When asked for your business type, select: ☑️ Sole Proprietor

This ensures you're guided through the correct documentation and requirements.

Step 2: Provide Business Information

  • Business Name: Use your registered business name. The details should always match the registration documents.

  • Industry Category: Select which category applies closely to your products.

  • Date of registration

  • Business Address

Step 3: Provide your details

Make sure the ID is:

  • Not expired

  • Fully visible (all edges and information clear)

  • Matches the name you entered

  • You can use one (1) of any primary ID:

    • National ID/PhilSys ID (Physical) - Front and Back sides

    • National ID/PhilSys ID (Digital)

    • Driver's License

    • Passport

    • SSS / GSIS

    • UMID

    • ACR / Immigrant COR

    • PRC

    • Postal ID

    • Voter's ID

For identity verification, you'll also need to upload:

  • A selfie holding the same ID you uploaded

  • Your face and the ID must both be visible

Step 4: Business Documents

Please ensure that the name on all submitted documents matches the legal name you provided on the form. We strongly recommend that you submit non-expired documents and verify that all uploaded images are clear, complete, and easy to read.

  • BIR 2303

    • The document must contain your business TIN (Tax Identification Number)

    • The document must be stamped on the last page

    • Make sure you upload all pages of the document

  • Notarized Secretary's Certificate

    • Please download this template Secretary's Certificate.docx

    • This document must be signed by the Corporate Secretary and notarized by a public notary

    • This document ideally includes the statement that your company is
      authorizing the opening of an account in Xendit and naming a person to sign the relevant agreement with Xendit

    • The business name and the authorized signatory's name must be clear and readable

    • All pages of the document must be submitted

  • SEC Certificate of Registration

    • Should include entity Name, Purpose, and Business

    • The address should be clear and visible.

    • QR Code can be seen in the document.

    • The document should be signed by the directors.
