Here's a step-by-step guide to help you fill out the form correctly and avoid delays in your verification process.
Step 1: Choose the Correct Business Type
When asked for your business type, select: ☑️ Sole Proprietor
This ensures you're guided through the correct documentation and requirements.
Step 2: Provide Business Information
Business Name: Use your registered business name. The details should always match the registration documents.
Industry Category: Select which category applies closely to your products.
Date of registration
Business Address
Step 3: Provide your details
Make sure the ID is:
Not expired
Fully visible (all edges and information clear)
Matches the name you entered
You can use one (1) of any primary ID:
National ID/PhilSys ID (Physical) - Front and Back sides
National ID/PhilSys ID (Digital)
Driver's License
Passport
SSS / GSIS
UMID
ACR / Immigrant COR
PRC
Postal ID
Voter's ID
For identity verification, you'll also need to upload:
A selfie holding the same ID you uploaded
Your face and the ID must both be visible
Step 4: Business Documents
Please ensure that the name on all submitted documents matches the legal name you provided on the form. We strongly recommend that you submit non-expired documents and verify that all uploaded images are clear, complete, and easy to read.

The document must be stamped on the last page
